Commitment to safety: All of us – staff, families, and young people - share the responsibility to keep our West End House community safe. We have established a strong safety plan based on CDC guidelines with the goal of reducing our exposure to and transmission of COVID-19. Here are some of the safety requirements at a glance:

I will check each day to see if my child has any COVID 19 symptoms
I will keep my child home if he or she exhibits any of the COVID 19 symptoms and call the West End House to report. I will contact the WEH if I am notified by the Department of Public Health that anyone in my family has been exposed to COVID 19.
If my child is home sick and has a COVID 19 symptom, I understand I must get a COVID 19 test with a negative result before coming back to West End House. Also, if my child is sent home from the West End House with a COVID 19 symptom, I understand I must get a negative test result before coming back to West End House.
I certify that the application is factual and complete to the best of my ability. I hereby give permission for the minor child listed on the application to become a member of the West End House. I understand the WEH is not responsible for personal injury or loss of personal property and that I will be financially responsible for any intentional damage or vandalism to the WEH caused by my child. It is understood that the code of conduct will be adhered to and a member’s failure to comply may result in membership termination.
I, the parent/guardian of the minor child listed on this application, on behalf of the minor child listed herein and for ourselves, our heirs, executors and administrators, hereby release, waive, acquit and forever discharge the WEH, their representatives, successors, insurers, assigns or any other person or entity associated with any of the above organizations such as staff, directors or volunteers, from all liability, claims, demands, or causes of action for any and all loss, damage, injury or death and any claim of damages resulting from use of facilities owned or controlled by the above organizations, or participation in activities of said organizations either at or away from the WEH.
