    • **Basic Instructions** 
    • INCIDENT OCCURRENCE REPORTING BASIC INSTRUCTIONS:

      All the reporting criteria is included in one single form, and it must be reported through the online Safety Reporting portal. These must be reported according to two main categories, Aviation Safety Occurrence and Other Safety Occurrence.

      FOR AVIATION SAFETY OCCURRENCES – (AIRCRAFT INVOLVED):

      • FIRST – Immediately after the scene is secured, initieate reporting procedures. Access the ERP Binder, call the Operations Manager, and follow the given instructions. (In the unlikely event that there is no answer, leave a detailed message and call the Director of Maintenance or the SMS Coordinator/Health & Safety Representative).
      • SECOND – submit a detailed online Safety Reporting Form ASAP. Select the proper reporting criteria and follow the instructions.
      • THIRD – Adhere to any investigation reporting requirements in a timely manner.

      FOR OTHER SAFETY OCCURRENCES:

      • FIRST – Immediately after the scene is secured, initieate reporting procedures. If needed, access the ERP Binder, and follow the corresponding guidelines.
      • SECOND – submit a detailed online Safety Reporting Form ASAP. Select the proper reporting criteria and follow the instructions.
      • THIRD - Adhere to any investigation reporting requirements in a timely manner.

      AUTOMATED NOTIFICATIONS:

      The online Safety Reporting Form system will automatically notify the appropriate chain of command depending on the selected safety reporting criteria. If the online Safety Reporting Portal is not accessible, a paper copy of the Safety Reporting Form is available in the SMS binder-Section 25, or a printable version is available in the Safety Library site under the Safety Forms tab.

      Please Note: Any neglect or intention to not adhere to the Safety Reporting Policy, without a valid reason, will result in corrective disciplinary action including, but not limited to termination of employment.

      NOTE: Depending on the type and magnitude of the incident, Transport Canada and/or Transportation Safety Board of Canada may require their own reporting requirements.

      We are confident that this approach will provide a collaboratively safe, supportive, and constructive safe work and business environment for all persons employed or served by L R HELICOPTERS INC. However, at no time shall any L R HELICOPTERS INC. company policy in any company manual take precedence over applicable government legislation, with which all workers must be familiar.

      If this is a harassment and/or violence occurrence, you must go to the Harassment & Violence Reporting Form to file a notice of occurrence as per regulations and company safety policy (In continues development, but ready to be used).

      All current safety and administrative reporting practices will undergo constant updates and renovations. This is to:

      2. have access to a more productive and user-friendly tracking system,
      3. promote a more inclusive working environment,
      4. improve all channels of communications,
      5. follow new and updated Federal regulated law, and
