This form is for volunteers who are interested in becoming models for different services. All models are selected based on the needs of the artist and class requirements. There are different health conditions that must be made aware to the provider prior to the service. Selected services require the model to be 18 years of age or older. All models will need to sign consent forms prior to the service completion. Any work completed by students will be supervised by a trained technician.
