AGREEMENT: (Please read and sign)
ELEVATE is a ten-week online class designed to motivate and inspire you to discover and live your best life. The goals are to promote and build self-awareness; cultivate self-love and compassion; grow a sense of belonging while nurturing autonomy and empower your ability to sustain joy. We will do this together by working one on one with the facilitator, interactive group work with other participants, and motivated personal reflection. While every lesson is designed to achieve these goals, individual experience will vary, and guarantees are not implied.
This class is not therapy or counseling. However, it may address specific personal projects or situations, business successes/difficulties, or general conditions in your life or profession. Although mostly supportive, it can also be challenging. Even though the facilitator (Leslyn Kantner) is a trained and licensed psychotherapist, no therapeutic alliance is to be assumed or implied. The facilitator is implicitly prevented from engaging in a therapeutic relationship with any class participant during the program course. If issues arise that are best dealt with in a psychotherapy context, a referral will be made to an appropriately licensed therapist or facility.
Upon registration of ELEVATE and signing this agreement, you are agreeing that if any mental health difficulties arise during the class, you will notify the facilitator immediately so that you can be provided with an appropriate referral.
Payment:
The fee for ten weeks of participation in ELEVATE is $750. Payment must be made prior to the first class. Should you be unable to complete the program you have two options:
Discontinue BEFORE third class and receive a 50% refund of the fee paid.
Discontinue AFTER third class and receive the right to step back in where you left on in another group when space is available for $150.
Another option is to AUDIT the class for a fee of $99.00. Audit patrons will have full access to The Elevate Class classroom and syllabus. No other elements of the class will be accessible. If - at any time within 12 mos of registering as an audit patron - an individual wishes to experience the class in its entireity - the audit fee may be applied to THE Elavate Class tuition.
Confidentiality & Legal Issues
In general, all of the information discussed during private sessions with the facilitator is confidential unless (1) you give specific permission to release the information or (2) the facilitator is required to release the information by law. Exceptions to confidentiality include: child, elder or dependent adult abuse; serious threats of hurting yourself or others; your condition indicates being a serious danger to yourself; your involvement in certain litigation processes; consultation with colleagues if needed, without identifying information included, and disclosure to collection agencies. Be aware that privilege may not apply under the auspice of motivational education. In other words, records may be subpoenaed. Distance communication, such as telephones (wired and wireless), email, and visual internet media such as Skype, may not be secure from eavesdropping, so if you agree to their use you are indicating your agreement to utilize a communication medium that may not be confidential. In addition, any comments or responses to groups cannot be guaranteed private to the group. Communication via social platforms (i.e., Facebook) are private only to the extent that the entity provides (closed group pages).
Participant agrees to indemnify, defend, and hold harmless the other party and its agents, officers, and employees from and against any and all liability expense, including defense costs and legal fees incurred in connection with claims for damages of any nature whatsoever, including but not limited to, bodily injury, death, personal injury, financial or businesses losses, or property damage arising from such party’s performance or failure to perform its obligations hereunder.
