The IndieCares Disaster Relief fund has been established to provide timely, fair, and equitable financial assistance to qualified independent pet retailers whose businesses have been negatively impacted by natural disasters. Disbursements are based on verified need and limited by available funds.
If you are an independent pet retailer who's business has been negatively impacted by a natureal disaster such as flood, hurricane, fire, or other act of nature, and you are seeking interim relief to cover costs not covered by other sources such as insurance, FEMA or other grants, please complete this application.
APPLICATION AND DOCUMENTATION
Applications must be completed in full, signed and include required documentation.
Incomplete applications will not be considered.
Additional documentation may be requested including: Proof of physical or operational damage and financial loss or hardship, proof of insurance, insurance claim denial, FEMA/local disaster report, or financial records.
