Before I can book you in for your tattoo, I need some relevant information from you! Please fill out this form as best as you can. If you aren't sure about your tattoo idea, or if you would prefer to discuss your ideas in person before committing to booking, I offer free consultation appointments - just email me to book: Lolly.tattoo@hotmail.com. I usually recommend this anyway if it is a large project, if there are any complex needs to discuss, if it is a cover up, or if you have any anxieties or concerns about any part of the tattoo process, so I can answer any questions you might have and ease any worries.
If you have ongoing work by me that you just want to continue, there is no need to fill out this form unless it is a completely new piece. Just email me to book an appointment.

If you would like a custom design, please include inspiration pictures to help illustrate your idea and style preferences. I’ll always create a new design for you and never directly copy another artist’s design (so please don’t ask) but it helps to see your taste and make sure we are on the same page design-wise. If you would like a specific design from my pre-drawn available designs, please include a picture or screenshot of the one you would like.

Please also include a photo of the area to be tattooed - please take pictures of the body part from a straight on angle - so either ask a friend to take the picture for you, or set up your phone on a selfie timer and step back. Be sure the body part is in a relaxed and natural position, and try and take the picture in an area with decent or natural lighting. You can include multiple pictures if you aren't sure you have taken the right one. Please include a few different angles/ views of the area if it is a cover up or if there are other tattoos nearby.

Your availability - let me know if there are certain days of the week that work best, or list dates that are good for you, having a few options helps a lot. My working days are Tuesday - Saturday.*

Booking in

Please note that in order to book a tattoo appointment, you will be required to pay a deposit to secure the booking, (usually between £50-£150 depending on the size of the tattoo) this is non-refundable and comes off the total price when you get the tattoo. The deposit can be paid via bank transfer after I have given you a price estimate and we have arranged a booking date.
