We create volunteer ID lanyards for all drivers. To assist us with this, please provide a current headshot/passport photo that clearly shows your face. This ID lanyard should be worn at all times when performing the duties of a volunteer driver.

EXPECTATIONS
Volunteers will:
- Make themselves available for their rostered Saturday of driving, with a start time of 1:30PM or 2PM depending on their assigned route and designated food collection point.
- Inform and communicate in a timely fashion so that the coordinators/team know if they are running late, or if they need to arrange a swap of rostered dates. Generally, the more notice the better.
- Comply with all road / driving rules and DHHS regulations pertaining to COVID-19 and the enforced lockdown restrictions. This includes ensuring that worker permits are in their possession during the period of volunteering.
- Follow the guidelines for delivery and appropriate level of contact with/acknowledgement from the recipient.
- Contact the Coordinator (or Point Leader) in the event that they encounter difficulties with the delivery experience or route challenges.
- Ensure hot foods (meals) are stored separately to the other foods (fresh eggs/frozen pies.
- Return any padded supermarket bags (collected from recipients) when coming to pick up meals for delivery.
- Advise (in the WhatsApp group chat) ASAP when they have completed their driving/food distribution.
CONSENT TO HOLD INFORMATION

I consent to the information contained in this application to be kept by Love Loud. As the coordination partner, this means that Clayton Church of Christ Fellowship (the church) may also hold this information on file. I understand that this information will be kept confidential and used only for Love Loud Meal Delivery coordination purposes.

DECLARATION
1. I understand that Love Loud and the church operates in an environment of numerous legal and ethical restrictions, and I will fully cooperate with the church in abiding by these. I assure Love Loud/the church, in considering me for a volunteer role that:
  1. I have no health impediment that will put me or any other person at risk in the fulfilment of my designated role.
  2. Other than those matters disclosed by me to the church (noted above) at the time of making this declaration, I know of no past behaviour that renders me unfit to serve as a volunteer or which detracts from the obligation of the church to operate as a place of safety to a minor or any other person.
2. I have provided this information in good faith and declare they are true and correct to the best of my knowledge and belief. I understand that any material misstatement in or omission from this application is sufficient to cause my application to cease.
3. I will respect the decision of Love Loud as to whether I have been approved to be a volunteer.
