Authorization/Agreement

I do hereby request, authorize and give permission to the Page County Sheriff's Office, or duly authorized representatives, to act on behalf and in my stead, should my child complain of being ill, be injured or require emergency or other medical care, including hospitalization, during the Youth Football Camp day event. I understand that in the event my child complains of being ill or is injured during the event, he/she will be looked at by EMTs and transported to the Page Valley Memorial Emergency Room, if necessary.

I agree to hold harmless, the Page County Sheriff's Office, its deputies, directors, employees, administrators, agents, successors and assigns from all claims, demands, damages, actions or causes of action, present or future, whether known, anticipated or unanticipated, and resulting from, rising out of, or incident to their actions pursuant to this authorization.
