If your file is too large fopr the jotform please still fill out this form but send your video file (with audio included in the video) to music@lushmpls.com and include your name and date of show.
After 3 entries, and we are filling uo the weekly slots, PRIORITY WILL BE GIVEN TO PERFORMERS WHO HAVE NOT YET GOTTEN A SLOT. :)
