Event: MAD Elite Fall Fest – Haunted Pathway
IMPORTANT HEALTH & SAFETY WARNING
The Haunted Pathway is an immersive, theatrical experience designed to frighten and surprise participants. The attraction may include:
Flashing lights, strobe lights, lasers, and sudden changes in lighting
Artificial fog, haze, and reduced visibility
Loud music, sound effects, screaming, and sudden noises
Jump scares, costumed actors, and frightening imagery
Dark or confined spaces, uneven surfaces, and obstacles
Other theatrical effects that may cause fear, disorientation, or discomfort
HEALTH ADVISORY: This attraction may not be suitable for individuals with epilepsy, seizure disorders, photosensitivity, asthma, respiratory conditions, heart conditions, sensitivity to fog or loud noises, or other medical conditions that may be aggravated by these effects.
Participants who are pregnant, have mobility or balance limitations, or are especially sensitive to frightening experiences should carefully consider whether to enter.
ASSUMPTION OF RISK
I understand that participation in the Haunted Pathway is voluntary and involves inherent risks, including but not limited to fright, anxiety, dizziness, falls, trips, collisions, and adverse reactions to lighting, fog, sound, or theatrical effects.
I voluntarily choose to participate, or permit my minor child to participate, with knowledge of these risks.
RELEASE OF LIABILITY & HOLD HARMLESS AGREEMENT
To the fullest extent permitted by Virginia law, I agree to release, indemnify, and hold harmless MAD Elite Dance Company, its owners, directors, employees, instructors, volunteers, performers, agents, and event representatives from claims, demands, damages, losses, or expenses arising from participation in the Haunted Pathway, including claims involving ordinary negligence, except where such release or indemnification is prohibited by applicable law.
I understand that this agreement does not waive rights or claims that cannot legally be waived.
PARTICIPANT RULES & ACKNOWLEDGMENT
By signing below, I acknowledge and agree that:
I have read and understand the health and safety warnings.
Participation is voluntary, and I may request to exit the attraction at any time.
I will not run, push, shove, touch performers, or intentionally damage decorations or equipment.
I will follow all instructions provided by event staff and volunteers.
I understand that failure to follow safety rules may result in removal without refund.
I am responsible for determining whether I or my child can safely participate.
I understand that the attraction includes intentionally frightening effects.
