1. Activity Supervisors
Bonnie Kraxberger, Executive Director of Musical Exploration and Learning. Individual teachers may be assigned as appropriate. All staff at Maestro Music Academy have completed background checks and undergo yearly safety training. Substitute teachers may be assigned on an as-needed basis to accommodate performance schedules and illness. Advance notice will be provided and rescheduling offered as available.

2a. Private Lesson Tuition
Tuition payment is due in full on the first of every month, and is considered late if not received prior to the fifth of the month. A late fee of $5 per day will be assessed if my account is not current. No refunds are available at any time. A deposit of $50 is due at the time we reserve a lesson and an instructor for my student. This deposit is held until such a time that you, our staff, or we mutually decide to terminate instruction for your student.Termination by you requires two weeks advance notice, and the deposit will be applied to any outstanding balance at time of termination whether or not adequate notice was provided. Student must attend the final lesson at the end of the two week period to be eligible. The private lesson cancellation policy is 24 hours and that if advance notice is not given for any reason (including illness or schedule changes), credit will not be given for the lesson and that it will be marked as ‘missed’, or when the student fails to arrive within 10 minutes of the scheduled start time. I understand that Maestro Music Inc does not offer any refunds at any time, and that any lessons needing to be rescheduled will have a lesson credit that may be applied to a future lesson within 30 days only.
2b. Group Class Tuition
Tuition payment is due before the start of class. Participants should request an invoice for either one trial class, a punch card of 10 classes in Beaverton, or register & pay through the Sandy Community Center. Please note that new material will be covered during every class and that review time will be offered during the first five minutes to assist students in maintaining musical progress. No deposit or advance notice is required for this class format, however tuition must be current in order to attend class. No make ups are available for group classes. RSVPs appreciated to assist in class planning.

I certify that I will be responsible for any and all books, games, CDs, instruments, and any other materials or supplies checked out to me or my child by Maestro Music Inc. I understand that materials are due back within 30 days of a request to return, or no later than the final lesson or class with Maestro Music if my student discontinues lessons. I will return them in the same condition in which they were lent. I understand that if for any reason the materials are significantly damaged* or lost that I will be billed for the full replacement value of the item. *Significantly damaged materials include but are not limited to the following examples: books missing pages, binding, or cover; books with pen or other permanent marks inside or outside; books with pages folded, dog eared, or torn; CDs with scratches or damage to the cover. Copyright laws prohibit all photocopying of materials and that all printed documents issued by Maestro Music Inc follow and comply with all applicable copyright laws.

4. Health Requirements
Do not bring your student to Maestro Music if they display any signs of illness, including but not limited to coughing, sneezing, sore throat, vomiting, nausea, fever, rashes, or diarrhea. We reserve the right to send any sick student home without advance notice and without offering credit for the lesson.

If the above-named person requires any emergency medical treatment or procedures during the activities, I hereby consent to and authorize the above-named activity supervisor(s) to make any decision and take any action to arrange for such procedures or treatments in the discretion of the activity supervisor(s). I will be notified as soon as possible in the event of a medical emergency.
I release and waive, and further agree to indemnify, hold harmless or reimburse Maestro Music Inc., the individual members, agents, employees and representatives thereof, as well as activity supervisors, from and against, any claim which I, any other parent or guardian, any sibling, any child, or any other person, firm or corporation may have or claim to have, known or unknown, directly or indirectly, for any losses, damages or injuries arising out of, during, or in connection with the child's participation in the activities (including all forms of transportation) or the rendering of emergency medical procedures or treatment, if any.
