I agree that the nature and purpose of the treatment has been explained to me, and any questions I have regarding the treatment have been explained to my satisfaction.

I understand that with any treatment certain risks are involved and that any complications from known or unknown causes could occur.
I understand that possible side effects include, but are not limited to: mild to moderate redness, mild to moderate peeling or flaking, stinging, dry skin, tenderness, temporary irritation, pimples, cold sores or allergic reactions.

I acknowledge that if I am allergic to one or more ingredients in the products used, I may experience allergic reactions. Most side effects are temporary and will dissipate usually within 3-7 days.

I understand that it is recommended prior to having a facial infusion or any facial treatment to not have used Retin A for 72 hours, Accutane in 6 months, or have waxed 24 hours prior to receiving treatment. 

I acknowledge that I should avoid the use of Retin-A type products, aggressive exfoliation, waxing, and products containing acids that are no part of the recommended take-home regimen for 2-4 weeks following treatment.

I acknowledge that if I fail to apply minimal sunscreen (spf 30 UVA&UVB protection), I am more susceptible to sunburn, skin damage, and hyperpigmentation. I should avoid direct sunlight after a treatment.

I consent (to the best of my knowledge) that the answers I have given are correct and that I have not withheld any information that may be relevant to my treatment.

I acknowledge that this treatment is strictly elective cosmetic procedure and no medical claims have been expressed or implied. I give consent for all future treatments.

I will call to inform my skincare professional of any complications or concerns I may have as soon as they occur. 

I release Skin By Mal and its staff of any liability associated with any injuries and or current and future conditions resulting from the skincare procedures or products.
