Use this form if you are claiming money back from MKP SA, or requesting an advance for MKP SA related spending. All refunds will be made by direct bank deposit.

Please ensure that all invoices, receipt and/or vouchers are included.

This request will first be sent to the budget holder for approval before payment is made.

For NWTA-related payments, the budget holder is the weekend coordinator, or his nominated bursar.

For non-NWTA regional payments, the budget holder is the regional centre director.

For all other payments, the budget holder is the national treasurer.
