NOTE: If you choose not to register electronically for classes and sign this Waiver of Liability and Hold Harmless Agreement, please fill out Samba with Maria 2025 Samba Parade Dance Class Registration and Waiver of Liability and Hold Harmless Agreement (Print Version), sign it and submit it with payment at your first class.

Class Information

This year's parade dance classes will occur on Thursdays, 5:30-7:00 p.m., at two venues:

7/24-8/7 at Redwood Raks Collective World Dance Studio, 824 L Street #16, Arcata

8/14-9/18 at McKinleyville Activity Center, 1705 Gwin Road, McKinleyville

To register for classes, please do all of the following:

1. Read ALL of this form and the linked Redwood Raks Collective World Dance Studio Registration & Liability Release Form (You don't need to fill out the form.)

2. Enter the information collected on this form

3. Sign this Agreement electronically and submit it online by July 23 at 5 p.m. You will receive a copy of the completed Agreement after you submit it.

4. Pay for class

9-Class Parade Session = $135

OR

Single-Class Drop In Fee = $17 (To participate in the parade, you must attend at least attend 6 classes and know the parade choreography)

IMPORTANT NOTE: This Registration and Waiver of Liability and Hold Harmless Agreement is only for parade dance classes. You do not have to participate in the parade to attend classes. To participate in the parade, you must register and order your costume separately through Samba do Mar Humboldt. Parade E-Registration and Costume Information will be made available on Sunday, July 27. Registration and Costume Fees will be due no later than Saturday, August 2. We will be available on Thursday, July 31, after class from 7:30 - 9:00 PM at Slice of Humboldt Pie to help with measurements and costume questions.

LIABILITY RELEASE:

Waiver of Liability

I understand that my participation or my child's participation in Samba with Maria Vanderhorst dance classes and dance-related activities present a risk of injury. I understand that there is an inherent risk of injury that cannot be eliminated regardless of the care taken to avoid injury. I agree to assume any and all risks of injury or death, from any cause or source whatsoever in order to participate in or allow my child to participate in these activities.

I do hereby release, waive and forever discharge Maria Vanderhorst, Redwood Raks Collective, McKinlyeville Community Services District, Samba do Mar Humboldt, Ink People Center for the Arts, any substitute dance teacher, their predecessors and successors, employees, owners, agents, and assigns, and all other persons, corporations, and entities from all claims, expenses, attorney fees, and causes of action or suits of any kind or nature associated with my or my child's involvement with any classes, shows, or any other associated activities at or associated with Samba with Maria Vanderhorst, Samba do Mar Humboldt and Redwood Raks World Dance Studio/Redwood Raks Collective.

Protection of Property
I understand and agree that it is my or my child's sole responsibility to safeguard my personal property while attending or participating in any classes, rehearsals, workshops, or performances. I hereby release Maria Vanderhorst, Redwood Raks Collective, McKinlyeville Community Services District, Samba do Mar Humboldt, Ink People, any substitute dance teacher, their predecessors and successors, employees, owners, agents, and assigns, and all other persons, corporations, and entities from all liability for loss or damage to my personal property while attending or participating in classes, rehearsals, workshops, or performances. I or my child also agrees to abide by any rules, regulations and policies set forth by Maria Vanderhorst, Redwood Raks Collective, McKinleyville Community Services District, Samba do Mar Humboldt or The Ink People. 

Medical Attention
In case of physical injury or medical emergency, I hereby authorize Studio Danza to make necessary arrangements to transport myself or my child to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense. In extreme emergency, or if my child is under 18 years of age, I understand that Maria Vanderhorst or Samba do Mar Humboldt will attempt to notify the person(s) I have named as my emergency contact(s) of
my or my child’s condition and how to reach me.

Photo Release
Maria Vanderhorst and Samba do Mar Humboldt reserve the right to use photographs and videos taken during classes, workshops, performances, or other affiliated events for the purposes of instruction, advertising and promoting Samba with Maria, Samba do Mar Humboldt and their programs. Students, or parents/legal guardians of students who are minors, who do not wish to comply with this
policy must notify Maria Vanderhorst or Samba do Mar Humboldt prior to participation in class. 

Acknowledgement of Waiver
In signing this Agreement, I acknowledge and represent that I have fully read and understand the content of this WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT and THE REDWOOD RAKS COLLECTIVE WORLD DANCE STUDIO REGISTRATION & LIABILITY RELEASE (Please click the link. You do not have to enter information in the form.) before I sign this Agreement. I understand that I sign this Agreement as my own free act and deed; no oral representations, statements, or inducements, apart from the written statement, have been made. I further state that I am at least eighteen (18) years of age and am fully competent to sign this agreement; and that I execute this release for full, adequate, and complete consideration fully intending to be bound by the same. I further state that there are no health-related reasons or problems which preclude or restrict
my or my child’s participation in this activity, and that I will pay any medical costs that may be attendant as a result of injury to me or my child.

Electronic Signature Statement:

I agree that this Agreement may be electronically signed. I agree that the electronic signature appearing on this Agreement are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility.
