  • 1.) COMMITMENT

    You are expected to follow the training and nutrition plans provided to the best of your ability to achieve the desired results. Consistency and communication are key to success.

    2.) PAYMENTS

    All coaching fees must be paid in full before the start of your program. Fees are non-refundable, except in cases of documented medical conditions preventing participation.

    3.) LIABILITY

    You acknowledge that all physical activity carries inherent risks. You are responsible for ensuring you are physically fit to engage in the training program.

    4.) CANCELLATIONS AND RESCHEDULING

    For check-ins/calls or personal training sessions, cancellations must be made at least 24 hours in advance. Missed calls/Personal training sessions without prior notice will not be rescheduled.

    5.) PRIVACY

    All personal information shared will be kept confidential and used solely for the purpose of creating personalized fitness and nutrition plans.

    6.) CONFIDENTIALITY

    All the information on this form is correct and to the best of my knowledge. I have sought and followed any necessary medical advice. I understand that all the information given will be kept confidential.
