I understand that I need to contact my veterinarian and let them know that they have my permission to give McNeely Farmily Rescue and Sanctuary any needed information relative to the foster/adoption process. And that by not doing so will slow down the process.

I understand that if the foster animal needs a veterinary appointment for an issue that I will contact McNeely Farmily Rescue and Sanctuary so that they may make an appointment with the rescue's veterinarian. Transport to and from may be negotiated by both parties, however the responsibility falls on the rescue.

Are you willing to take your foster dog or cat to vet appointments at a convenient time for you?

Are you willing and able to medicate your foster, (with provided medications with instructions should it need them) even if it just a monthly heartworm preventative?

We cannot guarantee a dog to be housebroken, are you equipped to train with love and patience?

I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any McNeely Farmily Rescue and Sanctuary animal fostered by me. I certify that the above information is correct, and I understand that the information will be verified. I understand that by submitting this form electronically, I agree to release and covenant to hold harmless McNeely Farmily Rescue and Sanctuary and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster dog or cat. I accept full responsibility for the dog(s) or cat(s) actions at all times, and release McNeely Farmily Rescue and Sanctuary from any liabilities or damages that may be incurred because of fostering such dog(s) or cat(s). I agree to have McNeely Farmily Rescue and Sanctuary complete reference call checks and conduct a home visit inspection to be able to approve my foster application. I agree that if I'm unable to foster the dog(s) or cat(s) anymore that I will return the dog(s) or cat(s) to McNeely Farmily Rescue and Sanctuary and try to give McNeely Farmily Rescue and Sanctuary a maximum 2 week period to try and find another suitable foster for the dog(s) or cat(s).
