Instructions (Annual & Conclave Authorized Vendor Form)
This form must be completed by individual members, affiliate chapters, or other entities requesting authorization to serve as vendors for Groove Phi Groove®.
Authorization may apply to one of the following options based on the fee selected and approved:
International Conclave Vendor Authorization – Permission to sell Groove Phi Groove® paraphernalia, products, or services exclusively during the Annual International Conclave, which is typically held during the second or third week of July each year.
Fiscal Year Authorized Vendor Approval – Permission to sell Groove Phi Groove®-approved products or services throughout the fiscal year (September 1 – August 31).
Only vendors who have been approved and have paid the applicable Authorized Vendor Fee will be permitted to sell Groove Phi Groove® products and services.
Payment Instructions
All payments must be made by:
Money Order
Check
Credit Card (MasterCard® or VISA® only)
Do Not Mail Cash.
IMPORTANT NOTICE
If you are submitting payment by money order or check, do not complete this form online. Please mail your payment directly to the designated address listed on the applicable Vendor Payment Instructions and contact the International Office for confirmation and processing.
Make all money orders or checks payable to:

Include in the memo line:
“Conclave Vendor Fee” (if applying for Conclave only)
“Authorized Vendor Fee” (if applying for Fiscal Year Authorization)
