I understand that by submitting this form electronically, I agree to release and covenant to hold harmless RDK9 and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster dog.

I accept full responsibility for the dog(s) actions at all times, and release RDK9 from any liabilities or damages that may be incurred because of adopting such dog(s).

I agree to have RDK9 complete reference call checks and conduct a home visit inspection to be able to approve my adoption application.

ADOPTION CONTRACT

The following is an adoption contract for dogs adopted through 501c3 Regional Deployed K9 & Equine Rescue. Please be aware that no applicant will be approved without: photos of their accommodations being sent to regionaldeployedk9@gmail.com, references being checked, and a home visit scheduled and completed by RDK9 if it's possible. Please allow a minimum of 48 hours for your application to be reviewed prior to contacting RDK9 regarding approval.

I understand that by signing this adoption contract I will take full responsibility for the financial costs and care of the dog once I make my commitment for said dog. I agree that once I have committed to said dog that I will not abandon the dog with the rescue, in boarding, or once pulled by RDK9 if it has not been pulled yet and requires rescue tag. I also agree that I will not move, rehome, sell, or abandon said dog once in my possession and will contact RDK9 IMMEDIATELY if I can no longer keep said dog. RDK9 agrees to help find the dog a new home in the event that the dog can no longer be kept by the adopter but I understand and agree that I will be responsible for accommodations and costs associated with the return of the dog to RDK9 and no adoption fees will be reimbursed. I understand agree to give RDK9 no less then 30 days to attempt to find proper placement and accommodations for the dog. I also agree and understand that I am fully financially responsible for any boarding, care, accommodations for the dog if I can no longer keep the dog in my home during this time.

I understand that I am responsible for the daily costs & needs of the adopted animal including, but not limited to: food, water, crate, toys, collars, leashes, veterinary expenses etc.

I agree to provide the necessary daily care for the dog such as, but not limited to, providing proper and clean housing, proper and clean feeding and watering, proper exercise provided for the dog, necessary yearly inoculations and monthly preventatives such as for fleas, ticks, and heartworm and will provide proof of such if requested by RDK9.

I agree to spay or neuter the dog within 30 days of arrival at my residence if it has not been done prior to arrival and agree to never breed the adopted dog and will provide RDK9 with proof of spay or neuter within this time frame. Adoption will not be finalized until the animal has been altered. If the animal is younger then the appropriate age to be altered then I will provide proof of a future appointment for when the animal is no older then 1 year of age within 30 days of this contract being signed and I am approved to adopt. Again, I understand that until the animal is altered the adoption will not be FINALIZED. I understand that if I do not abide by these terms it is considered a breach of contract and Regional Deployed K9 Inc will have the right to repossess the dog and I will be responsible for all expenses associated with the repossession.

I also agree to seek immediate medical attention at my own expense should the dog become sick or injured.

I agree to never enter the dog into fighting rings or sell the dog to a breeder or pet store or abandon the dog at a shelter.

I understand that the dogs adopted out by RDK9 have come from a shelter and there are no guarantees given by RDK9 regarding but not limited to; breed, age, health conditions, and temperament. On the same note, I also agree that if the dog is flagged for aggression or poor behavior and I still wish to adopt, that I will not sue RDK9 or hold them responsible for the behavior of the dog. I agree to schedule a meet and greet with the dog available for adoption and my other animals prior to adoption if RDK9 feels it is necessary.

I agree to not euthanize the adopted dog for any reason without consulting with RDK9 and providing proper documentation by a licensed professional as to why I see the need to euthanize and RDK9 giving the final approval or denial. I agree that I will follow proper leash laws and keep the adopted dog properly contained whether by kennel or fenced in yard. I understand that if the dog escapes from my possession more than 3 times and ends up in a shelter I will be required to return the dog to RDK9 until proper adjustments to the property can be completed.

I understand that the microchip adopted dogs have will remain under the name of RDK9 and I will be added as a secondary contact to assure the safety of the adopted dog but that I will be its owner and responsible for any costs that ensue should said dog be picked up by animal control and taken to the shelter or if I can no longer keep. I also agree that should this happen; I will retrieve the dog within 48 hours of being notified that the dog is in a shelter or if found by a good citizen and held for return and will not abandon it. I also agree to not microchip the dog in my name.

Please Note: If adopting a dog that has been pulled by RDK9 but has not entered their property at the time of adoption they will provide as much information for the dog as they have been given but they cannot guarantee how the dog will behave once in your care. Also, RDK9 can only estimate the age and breed of the dog and cannot guarantee it is 100% accurate.

Please Note: Adoption costs vary per dog. If the dog is in boarding and has not been transported yet, you may be responsible for the cost of transport and boarding as the adoption fee. However, RDK9 will do their best to work with adopters on costs per situation.

I understand that if I do not abide by this contract that I will lose my right to adopt in the future, will have the animal confiscated, and could potentially face legal action taken against me.
