- I understand that lash extensions/lash lifts are a luxury to my lifestyle and that I am responsible for the aftercare and maintenance once the service is complete. (Aftercare instructions will be provided at the appointment.)
- Drinking coffee or tea before the lash appointment could cause twitchy and unsteady eyes. This may affect the lashing process and cause discomfort. Please refrain from drinking caffeine within 6 hours of your appointment.
- I understand that the nature of the services may require me to lie on my back, keep my eyes closed, and be mostly still for up to 3 hours. If extra back support is needed, it will be gladly provided upon request.
- I understand that talking extensively during the service may cause the eye to become sensitive and affect the final outcome.
- A deposit of $30 via e-transfer must be received before appointment is confirmed. This deposit will count towards the total at the time of service and is non-refundable. *Your appointment will not be secured until deposit is received.*
- ONLY CASH will be accepted at the appointment. *E-transfers may be accepted as an emergency but there will be a 5% GST add-on. NO EXCEPTIONS.*
- I have thoroughly read and understood the prices as listed in the menu.
- I understand that my lash artist may not be able to complete the service if I arrive late. Arriving more than 15 minutes late will result in an automatic cancelation and treated as a NO-SHOW. This will result in a charge of 50% of the service fee.
- If you need to reschedule or cancel your appointment, please do so with at least 24 hours notice. Failure to do so will result in a NO-SHOW fee of 50% of the service fee that must be paid before another booking can be made.
- I agree to arrive at my appointment with clean eyes, completely free of makeup, oils, lash glue, etc. If the hygiene level is unreasonably poor to work on, I will be asked to reschedule the appointment to another date. The appointment will be considered a last minute cancelation, resulting in the loss of deposit or a charge of $30 if deposit was not previously taken.
- If I NO-SHOW (3) times, Midheaven Lash and Beauty will no longer service me as a client.
- COVID-19 is still a great risk to many and there are so many reasons to continue wearing our masks. I understand that it is required for me to keep my mask on at all times possible, including before and after the service.

  This is a fully-vaccinated environment. Please understand that the studio is home-based and this is a precaution I am taking to ensure the safety of my family and other clients.

- I understand that Midheaven Lash and Beauty is a private home studio. I promise to treat the property with full respect.
- Please refrain from bringing any children, pets, parents, friends, or any guests with you to the appointment.
- During this time, restroom usage will be kept to a minimum as it is located in the main house and I would like to keep traffic to a minimum, if any. Please respect these rules and use the washroom before and/or after coming to your appointment.
- I understand that I may not be allowed entry unless an appointment has been made and confirmed. *Showing up at a time other than your appointment time will not grant you a sooner available spot and may interrupt another client's appointment.*
- I understand that Midheaven Lash and Beauty may refuse service at any time and that I may be asked to reschedule my appointment if above conditions are not respected. I understand that the $30 deposit will be forfeit if such a situation were to arise.
