· We understand that MVFC VE Day Cup Football Tournament is a contact sport. We acknowledge that the MVFC VE Day Cup Football Tournament is a physical activity which carries with it inherent risks of physical injury which cannot be eliminated completely.
· We expressly, knowingly, and voluntarily assume all of the risks of participating in the MVFC VE Day Cup Football Tournament. We understand and acknowledge that these risks include, but are not limited to falls, contact or collision with other participants and contact with natural and man-made fixed objects.
· We understand and acknowledge that these risks may cause harm including but not limited to bodily injury, ranging from minor to severe.
· We understand that the risks of participating in the MVFC VE Day Cup Football Tournament may be caused in whole or in part by our own actions or inactions, the actions, or inactions of other participants.
· We expressly assume all risks and take full responsibility for any and all damages, liabilities, losses, or expenses that we incur as a result of participating in the MVFC VE Day Cup Football Tournament
· We agree that it is our responsibility to determine whether we are sufficiently fit and healthy to safely participate in the MVFC VE Day Cup Football Tournament . We hereby represent that we are and on the date of the MVFC VE Day Cup Football Tournament will be, in good health and in proper physical condition to safely participate in the tournament. We certify that we have no known or knowable physical conditions that would affect our ability to participate safely in the MVFC VE Day Cup Football Tournament or that would result in my participation creating a risk of danger to ourselves or others. We further certify that we have not been advised or cautioned otherwise by a medical professional.
· We understand that the MVFC VE Day Cup Football Tournament is a physical activity. We understand that it is our responsibility to monitor our own physical condition during the session and agree to withdraw immediately and notify personnel if at any point our continued participation would create a risk of danger to ourselves or others.

I accept full responsibility on behalf of my team to fulfil all fixtures for the tournament. I accept that once the fixtures have been confirmed and teams allocated, if my team, cannot fulfil the fixtures we will not be refunded unless a replacement team is found to take their place. I agree to abide by the rules of the tournament and I, and all of my players are aged 18 or over.
