• Application Process:

    Completely fill out the application. Red asterisks are required fields; if a question does not apply fill in with N/A. It can take 7-14 days of submission to process your application. Please advise your references and veterinarian that a volunteer from Miller’s Cause will be contacting them. You must give prior permission to your vet to speak with us. Please take pictures of the inside and outside of your home to include the area’s dogs will be kept and the entire fenced perimeter including gates of your yard. Part of the application process is contacting your veterinarian, four personal references, a background check and home check. One attempt will be made to contact your veterinarian, if additional information is required, you will be required to provide that information to us. If your application is approved, a meet and greet will be scheduled. If that goes well, you will receive an agreement and invoice via email to complete and sign. Both must be completed and returned before arrangements will be made to move your new canine friend into your home. Transport arrangements vary and can have fees associated with them. Any transport fees will need to be paid in advance of the move. Please ask if you have questions. ***At this time are not transporting out of state. You must be willing and able to come pick up the dog. Also note our board requires 3 years of vetting history to include vaccinations and YEAR ROUND (12 months) heartworm prevention. Please ensure you have this before applying.*** Thank you.

  • A vet reference check is one of the most important aspects of the application process. We will not proceed with an application if it does not meet the minimum vetting requirement for current and past pets. This includes 3 years of vetting history to include vaccinations and year round (January through December) heartworm prevention.

  • I have read the above information carefully and have filled out this application honestly. I understand that omission of information and/or failure to answer all questions and sign the application can result in this application being declined. Also, if an omission or untruth is discovered after an adoption takes place, I understand that the Miller’s Cause reserves the right to annul the adoption and reclaim the animal. I give the rescue permission to fully investigate the information provided as well as contact veterinarians, personal references and complete a background check. If the application passes this review, I agree to provide home and yard pictures and an inspection before an adoption decision is made.

    In addition, I understand the adoption decision is dependent on many factors, including but not limited to the compatibility of the family and home to the individual animal, and other applications received on this animal. I understand it is the prerogative of Miller’s Cause to decide which home is most appropriate and that their decision is final. Unless otherwise indicated, I am free to ask that my application remain open for a future dog if my adoption on selected dog is not completed.
