Please note that the initiation fee for you and any additional members will be due at your office visit. Monthly payments for you and any additional members will be processed using the card on file that will be set up during your office visit.
Terms and Conditions
The Mint Freedom Plan provides enrolled members with discounted rates on a wide range of dental services offered by Mint Dentistry. Members are required to pay for dental services at the plan rate. Please be advised of the following terms: The plan is non-refundable and non-transferable. The initial enrollment fee is $150, with an additional $75 for each additional family member. A monthly membership fee of $15 per enrolled person is also required. To terminate your plan, you must email a cancellation notice to mintfreedomplan@mintdentistry.com at least 30 days before the next billing cycle starts. Failure to do so will result in automatic renewal of your plan and continued charges. For any questions about the Mint Freedom plan, please email mintfreedomplan@mintdentistry.com.
