As a foster, or foster family, I/we agree to the following:
1. The dog will continue to be called by, and referred to, by the name given by the rescue
2. A representative of MHSDR will be allowed to visit my premises to verify that the terms of this agreement are being followed.
3. The dog will be provided with a humane environment, including healthy, appropriate food as agreed upon by MHSDR to be fed daily in the morning and evening, or on a schedule agreed upon by MHSDR. I/we will provide clean and fresh water at all times, daily exercise, all medications as directed by MHSDR, clean bedding, and safe, clean, dry, temperate shelter indoors. The dog will only be crated if approved by MHSDR, in an approved crate, and only for the time frame approved by MHSDR. Diapers, belly bands, and other sanitary garments will be changed as needed to keep the dog clean and dry.
4. A safe, secure collar will be provided with I.D. tag to be worn at all times, including indoors. No choke chains or prong collars will be used.
5. I/we will provide appropriate food, toys, collars/harnesses/leashes, and bedding. MHSDR may provide these on a case-by-case basis. I/we will not purchase items that I/we will seek reimbursement for without advanced approval.
6. All applicable laws governing the care and custody of the dog will be obeyed.
7. The dog will be fostered as a companion only. The dog will not be put to any kind of work unless approved in advance by MHSDR.
8. The dog will never be left outdoors unsupervised for any amount of time. The dog will be exercised and taken for potty breaks outdoors on leash in a safe area and/or allowed off-leash in a clean, safe, securely fenced area only. If taken to a public area, appropriate caution with other animals and humans will be taken. Daily exercise will be provided consistent with the dog’s age and condition. No electric or shock devices may be placed on the dog without MHSDR advanced approval.
9. I/we will notify MHSDR IMMEDIATELY if the dog is lost and will respond immediately to all inquiries.
10. I/we will notify MHSDR of any medical concerns as soon as they arise. I/we will maintain sanitary grooming and provide medical care as needed. The dog will be transported to veterinary and grooming appointments, and adoption events as directed by MHSDR, or arrangements will be made by the foster with to provide transportation.
11. MHSDR retains ownership and responsibility for the dog. I/we may not make any legal or medical decisions regarding the dog unless in life-threatening emergencies. In emergencies, MHSDR will be notified as soon as possible, but not required before seeking veterinary care. MHSDR will reimburse Foster for reasonable emergency medical expenses. Non-emergency medical exams and treatments must be approved in advance by MHSDR and performed by a pre-approved veterinarian/hospital, or may be denied reimbursement. In most cases, MHSDR will arrange payment of expenses in advance.
12. Approved potential adopters will be allowed to meet the dog at our home or I/we will transport the dog to another reasonable location, following current health safety measures. I/we will answer all questions regarding the dog to facilitate adoption.
13. MHSDR inquiries about the dog and potential adopters will be responded to within 24 hours.
14. No smoking or vaping will be allowed inside the home.
15. If a pool, hot tub, or similar item is currently or subsequently installed, MHSDR will be notified and allowed to inspect for safety before the dog is allowed within 5 feet.
16. l/we understand that the dog is an animal, in a new environment, and can be unpredictable. At no time will we leave children under the age of 12 unsupervised with the dog.
17. I/we agree that MHSDR has sole discretion in determining what is healthy, humane, proper, appropriate, acceptable, reasonable, and what constitutes neglect and/or abuse.
18. I/we understand that should I wish to adopt the foster dog, I will be required to fill out an adoption form and be approved as an adopter by MHSDR.
19. I/we affirm that no member of my household has ever been convicted of an animal welfare law violation including but not limited to: neglect, cruelty, and/or abandonment.
20. MHSDR makes no representations, guarantees, or warranties about the age, breed, temperament, health, or behavior of the dog. If I/we have questions or concerns about any of the above, I /we will contact MHSDR immediately.
21. I/we agree to release MHSDR and any of its volunteers, representatives or officers, as well as the former owners of the dog, from any and all claims arising from injury or damage caused by the dog to people, other animals, and/or property.
22. I/we agree that MHSDR may require the return of the dog at any time and for any reason with impunity, and that the return of the dog will occur within the time frame set by MHSDR. Failure to return the dog in this time frame will result in MHSDR taking legal action to the full extent of the law.
23. I/we understand that by voluntarily signing this agreement, I/we are entering into a legal and binding contract with MHSDR. Breach of any term(s) of this agreement is deemed actionable by MHSDR to the full extent of the law.
