This is to authorize Plantation Bay Resort and Spa to charge to my account any and all charges incurred by the hotel because of my pet. I am taking sole full responsibility for my pet and releasing Plantation Bay Resort and Spa, its owners, managers, and employees from any and all liability, damage, fines, claims, losses, or cost of any type including attorney's fees, arising out of any claim for personal injury or property damage caused by my pet.
Furthermore, I will abide by the following guidelines:

That my pet has updated anti-rabies and/or vaccination shots, and a copy/photo of them will be provided to the hotel staff on or before our arrival date.
- That I will make sure that my pet is quiet, well-behaved, and non-destructive at all times. If I fail in this regard and my pet inconveniences other guests, I will be responsible for any and all costs associated with compensating the disturbed guest(s).
- That it is my responsibility to clean up after my pet, or I will be charged $200 nett per occurrence.
- That whenever we are in the common areas of the resort, my pet will be leashed or in a stroller/carrier.
- That my pet is allowed only in the outdoor section of the restaurants provided that I have a disposal bag and will clean up after my pet promptly.
- That my pet is not allowed to swim in any of the lagoons, natural beach, and freshwater pools, or be in the Spa or indoor shops/facilities.
- That I will not leave my pet unattended in the room at any time. Unattended dogs will be fined $100 nett and may be removed from the room by the manager on duty.
- That my pet is not allowed directly in hotel furniture, including beds, and that any damage to the furniture or room will be charged to the pet owner.
- That I have a cage/carrier in case of emergencies.
- That Plantation Bay Resort and Spa reserves the right to require room change(s) with applicable fees ($50 nett per room transfer), removal of dogs from the premises, and refusal or discontinue of service without refund if, in the hotel's sole discretion, the pet owner did not abide by these policies or the pet is considered unhealthy or dangerous, likely to frighten or harm other guests, or disrupt the hotel's operations.
