Personal Time Off Request Submission Policy:
Employees must submit this formal request for planned time off a minimum of 30 days in advance. If it is not possible to give such notice due to unexpected illness, or exemptions under the Alberta Employment Standards Act, the employee must communicate with their direct supervisor immediately.
Submitting this form does not guarantee the personal time off requested approval. Approval of the request will be provided by the employee's immediate supervisor via email within 7 days of the request being submitted.
