Dog Adoption Application

Hurricane Creek K9 takes in abused, neglected, and abandoned animals. Some dogs may not be suitable for placement in your home. Wa ask that you complete this appication so we can do our best to match you with the appropriate dog.

We make every attempt to screen our animals for health, behavior and temperament issues. Certain types of problems may not demonstrate themselves until the animal is in a home environment. For these reasons we cannot guarantee the behavior, health or temperament of any animal.

Our adoption fees are as follows:

Adult Dogs: $500.00 (includes spay/nueter, microchip and all vaccinations)

Puppies: $200.00 (includes microchip and all vaccinations)

*If a dog has pledges of support, the amount will be deducted from the fee.*
