1. The Church’s Personal Data Protection Policy is available for review at https://www.carmel.sg/personal-data-protection-policy/ (the “Policy”). The personal data you provide will be used for the purposes set out in the Policy including but not limited to the facilitation and administration of church services and activities that you will be participating in and to provide you with updates about such church services and activities.

A member of the Church may only vote by appointing the Chairman of the ACM as the member’s proxy to vote at the meeting. This proxy voting form is an instrument of appointment, also known as the Proxy Voting Form, and will be automatically forwarded to church@carmel.sg upon submission at the end of the form.

All completed proxy voting forms (electronic or postal hardcopy) must be submitted or reach church office not later than 30 December 2025, 2359hrs.

The Board of Elders commends to members the following 5 nominees for election and re-election to Session at our 2026 Annual Congregational Meeting.

长老会向会友推荐以下5位被提名人，参加新选委任和重选连任为长执会成员：

Dn Jerry Ng Kia Boon (Re-election as Deacon)
黄加文(重选连任)为长执团的执事
Proposer: Ps Jabez Chia  Seconder: Dn Terry Siow

Dn Jerry, attended Mount Carmel Sunday school in Lengkok Bahru as a young boy. He worshipped in Life BP Church during his teenage years and was baptised in New Life BP Church. For the past 25 years, he has been worshipping at Mount Carmel. Since 2008, Jerry has served as a worship leader and currently leads the Alpha Ministry. He is passionate about presenting the gospel to non-Christians and promoting Christian education among believers. Jerry works in the IT department of a local bank. He is married to Chloe, and they have two children, Kathleen and Klarisse. If re-elected, Jerry will continue to serve as Deacon overseeing the Alpha Ministry.

Dn Adrian Wong Wei Ern (Re-election as Deacon)

Dn Adrian was baptised in Carmel as an infant on 24 December 1979 and has been worshipping with us ever since. He reconfirmed his faith on 22 December 1996. Adrian, a lawyer, currently serves as the Clerk of Session. He also serves as a musician and worship leader. He sat on many church camp committees over the years. He is married to Madelaine, and they have three children: Mark, Lauren, and Jude. If re-elected, Adrian will continue to serve as the Clerk of Session.

Mr. John Calvin Chua Zhi Hsien (Election as Deacon)

John Calvin, son of Pastor Daniel Chua and Phoebe, has attended Carmel since birth. He committed to follow Jesus during Youth Camp , after hearing the Gospel preached. He was called to full-time ministry while in university. Currently pursuing theological studies, he hopes to serve as a pastor in Carmel upon completion. John Calvin works part-time as a doctor and serves in the Young Adult Ministry (YAM), desiring to see young adults grow in love for Jesus, God’s Word, and community of faith. His passion is preaching, and his favourite book is the Gospel of John. He is married to Lynette with two sons, Micah and Emet. If elected, he will serve as the Deacon for YAM.

Mr. Joel Chia Zi Hao (Election as Deacon)

Proposer: Ps Oh Boon Leong   Seconder: Dn Titus Lau

Joel was baptised as an infant and has been in Carmel since birth. He reconfirmed his faith on 22 December 2019. Joel works as an economist at the Ministry of Home Affairs and serves actively in Carmel’s community service ministry and his CG. He is married to Shermayn. If elected, he hopes to serve in the Carmel Phase 4A Ministry and Evangelism Ministry.

Mr. Rayner Loi Kit (Election as Deacon)

Proposer: Eld Loh Pin Chuan   Seconder: Dn Benedict Leow

Rayner came Christ and baptised in 2011. He has been a regular worshipper at Carmel ever since. He started serving in the Youth Worship team and subsequently in the Youth Committee and Young Adults Ministry Committee. He is currently leading Rabah CG together with his wife. Outside of church, Rayner runs an AI tech start up tackling global food waste. He desires to steward his gifts and experiences to serve the body of Christ. He is married to Felicia. If elected, Rayner hopes to serve in the Phase 4A Committee, supporting the church’s plans in this next phase of growth and ministry.
