1. The Church’s Personal Data Protection Policy is available for review at https://www.carmel.sg/personal-data-protection-policy/ (the “Policy”). The personal data you provide will be used for the purposes set out in the Policy including but not limited to the facilitation and administration of church services and activities that you will be participating in and to provide you with updates about such church services and activities.

A member of the Church may only vote by appointing the Chairman of the AGM (Elder Bruce Lockhart) as the member’s proxy to vote at the meeting. This proxy voting form is the instrument of appointment and will be automatically forwarded to church@carmel.sg upon submission at the end of the form.

All completed proxy voting forms (electronic or postal hardcopy) must be submitted or reach church office not later than 21 May 2023, 2359hrs.
