VOLUNTEER RELEASE AND WAIVER OF LIABILITY

By submitting this agreement, the Volunteer agrees to the following:
This **Release and Waiver of Liability** (“Release”) is executed on the date of submission by the undersigned volunteer (the “Volunteer”) in favor of **Music Changing Lives (MCL)**, its affiliates, partners, funders, sponsors, and their respective officers, directors, employees, volunteers, agents, and assigns (collectively, the “Released Parties”).
The Volunteer desires to work as a volunteer for MCL and engage in activities related to volunteering. In consideration of being permitted to volunteer, the Volunteer freely executes this Release under the following terms:
1. Waiver and Release
The Volunteer does hereby release and forever discharge MCL and the Released Parties from any and all liability, claims, demands, or causes of action, known or unknown, in law or equity, that arise or may hereafter arise from Volunteer’s activities with MCL.
The Volunteer understands and acknowledges that this Release discharges MCL from any liability or claim with respect to any bodily injury, personal injury, illness, death, or property damage that may result from Volunteer’s work with MCL, whether caused by negligence of MCL or otherwise. This Release is intended to be as broad and inclusive as permitted by the laws of the State of California.
2. Assumption of Risk
The Volunteer understands that activities with MCL may include physical activity, travel, community events, art projects, food distributions, or other volunteer efforts that carry inherent risks. The Volunteer knowingly assumes the risk of injury, illness, death, or property damage that may result from such activities.
3. Code of Conduct & Harassment Policy
MCL strictly prohibits harassment, discrimination, or misconduct of any kind. Volunteers agree to maintain respectful conduct at all times. Any violation should be reported immediately to MCL leadership, and appropriate action will be taken.
4. Photographic & Media Release
The Volunteer grants MCL unrestricted rights to use photographs, video, audio, or other recordings of the Volunteer for lawful purposes, including but not limited to promotional materials, reports, and social media, without expectation of compensation.
5. Medical Treatment
The Volunteer releases MCL from any claim arising from first aid, treatment, or medical service rendered in connection with Volunteer’s activities, whether provided by MCL or others.
6. Insurance
The Volunteer understands that MCL does not provide health or accident insurance for volunteers. Each Volunteer is expected to carry their own medical insurance coverage.
7. Governing Law
This Release shall be governed by and construed in accordance with the laws of the State of California.
BY SUBMITTING THIS FORM, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD THIS RELEASE, AND THAT I VOLUNTARILY AGREE TO BE BOUND BY ITS TERMS.**
