Applicant(s) herein agree that by submitting this application they hereby agree and direct the Veterinarian and Vet Clinic listed herein to release all medical history and information to Must Luv Dogs Rescue, their representatives, in order to qualify for an adoption or as a foster home for a rescued dog.

Applicant(s) herein also acknowledge that selection and approval of a family for a dog in the care of Must Luv Dogs Rescue ("MLD") is solely at the discretion of MLD, it's director, officers and agents. MLD reserves the right to approve the family that is the best fit for each individual dog, based solely on the judgment of MLD. This decision is based on many factors, such as current living situation, family members, other pets in home, ages of children, etc. MLD does NOT discriminate based on race, sex or religious beliefs. All parties herein acknowledge that the dog's best interest comes first and foremost and MLD does not approve based on order of receipt of applications or any other factors except what is best for the dog, in their sole discretion.
