FTF SPORTS SUMMER BASKETBALL CAMP WAIVER & RELEASE FORM

1. Assumption of Risk
I acknowledge that participation in the FTF Sports Summer Basketball Camp involves physical activity, including running, jumping, and other athletic movements. I understand that such activities carry inherent risks, including but not limited to falls, collisions, sprains, fractures, and other potential injuries. By allowing my child to participate, I voluntarily assume all risks associated with camp activities.

2. Release of Liability
In consideration of my child being allowed to participate, I hereby release and discharge FTF Sports, its coaches, staff, volunteers, and representatives from any and all liability, claims, demands, or causes of action arising from any injury, accident, or mishap that may occur during camp activities, whether on or off the premises.

3. Personal Property
I understand and agree that FTF Sports is not responsible for any lost, stolen, or damaged personal items brought to the camp by participants.

4. Medical Authorization
In the event of an emergency, I authorize FTF Sports staff to secure medical treatment for my child if deemed necessary. I agree to be responsible for any medical expenses incurred.

5. Acknowledgment and Agreement
I have read and fully understand this waiver and release form. By signing below, I acknowledge that I am the parent or legal guardian of the participant and that I agree to the terms outlined above.
