Outreach Committee Grant Application
General Requirements:
1. All applications must be submitted no later than April 30th for the June 30 grant cycle or October 30th for the December 31st cycle.
2. All applications must be submitted in electronic format and include all required documents (listed below).
3. Please limit your grant request to no more than $3500.
4. Organizations receiving a grant must wait two years from receipt of those funds to become eligible to submit a new grant request application.
Before you begin, make sure you have the following documents ready to go! Please include your name and/or the title of the grant proposal in each file name.

You will be asked to upload the following documents:
1. Organization's tax exempt letter (if applicable)
2. Copies of the IRS federal tax exemption determination letters
3. The organization’s actual income and expense statement for the past fiscal year, identifying the organization’s principal sources of support
4. The organization’s projected income and expense budget for the current fiscal year, identifying the projected revenue sources
5. Budget of income and expenses. Program budget for program requests. Organizational budget for general operating requests.
6. Complete list of the organization’s officers and directors
