My Healthy Family

Please fill in the registration form below to let us know that you are interested in participating in My Healthy Family, a three-week hands-on, collaborative workshop in which parents and children learn together! At each workshop, families leave with a freshly-cooked meal. We will contact you to confirm that you are registered for the workshops you signed up for.

During My Healthy Family workshops, we help families get involved in cooking their own food, and encourage kids and parents to all take part! Curious about introducing your child to the world of cooking? Take the first step with us by participating with your kids!

The workshops will take place May 16th, 23rd, and 30th from 3:00 to 5:00

Demographic Information (optional)

*Please note, we collect this information in order to report back to funders and partners regarding attendance and participation.  It is very helpful to us to have this information.

However, we only report group information (for example, we might say "18 adults participated, 50% of whom were women and 60% of whom are over the age of 50").  Your personal information (including your name) is NEVER shared with other organizations or funders.

These workshops are targeted for families. Children are expected to be accompanied by an adult.

Please note - you can register a maximum of 1 adult per registration form.  If you would like to register more adults, please fill in another registration form.

 

Photo & video releases

Summary: By selecting YES to one or both of the boxes below, I agree to The Depot taking photos and/or recording videos of me. I agree to The Depot sharing these photos and/or videos, including for advertising and/or fundraising purposes, without receiving any monetary compensation.

Please note: You can participate in all Depot programs without agreeing to this media release; however, we ask that you remind staff that you do not want your picture taken if and when someone is taking pictures or videos.

Data collection consent

I agree that The Depot can collect and use my personal information in order to:

• Manage my (and my household’s) involvement in The Depot’s activities, including emergency food access, workshops, volunteering, other programs, and donating funds;
• Communicate with me about these activities, including making me aware of connected opportunities;
• Develop statistics and reports, which will be anonymized (no one’s personal information is included) and aggregated (using large numbers of people so that no individuals can be identified).  These are used to improve programs, seek funding, and develop advocacy tools.

I understand that I have the right to access the personal information that The Depot has collected from me, to correct it if needed, and to ask for it to be removed from The Depot’s system at any time. For more information about The Depot’s practices around personal information, please consult our Privacy Policy.

I agree this information will be kept for a maximum of seven (7) years after my last contact with The Depot, at which time it will be removed from The Depot’s systems.

I understand that I do not need to give The Depot my personal information, but this may have an impact on how The Depot services are delivered to me.

What is the Community Contribution Model?

The Community Contribution Model is a sliding scale payment system where participants give what they can afford for our programs. Contributions help The Depot keep programs accessible for everyone, while continuing to offer quality services and meeting growing demand.

The Blue, Yellow and Red levels are more subsidized and meant for community members living with lower incomes or more unstable financial situations. The Green levels are for community members in more stable or comfortable financial situations.

You choose the level that is right for you.

If you are unable to pay any amount, choose 0 under "choose your contribution amount" below.

The contribution amounts below are for the whole household, and include all workshops.
