Thank you so much for your interest in being a part of our guest chef series at the Resto Depot. In this series we hope to showcase the talents of our participants and share a piece of their culture with the community. Guest chefs are awarded a $200 honorarium, to be payed to the participant up to 3 weeks after the event takes place. Your recipes will also become a part of our programming, your name and signature dish will live on in the repertoire of dishes we serve at the Resto.

 

Selection:

• In order to be a candidate, please answer the following form.
• A team member will get back to you if you are chosen to participate.
• Please note that there is no guarantee for participation

 

Commitment:

• We require 3 hours of your time on the day BEFORE the event to prepare ingredients and work with volunteers
• We require 3 hours the morning your meal is served to facilitate making and serving the meal.
• Some emailing back and forth is necessary to discuss the meal and its preparations.
• We require you to fill out an evaluation form after serving your meal
• If the requirements of participation are not met in full, we may reduce the honorarium by 50 percent.
• You will recieve the honorarium AFTER filling out the evaluation form.

 

Fill out this form with the dishes you would like to present. We will reach out to selected quest chefs to select a dish, discuss operations, and schedule a date.

 

Food and Safety:

• The Depot may need to change quantities of different parts of the dish to match with our Healthy Food Policy. This will often include increasing the amount of whole grains or vegetables on the plate to make it closer to the Canada Food Guide's healthy plate.
• The Depot kitchen team is responsible for health and safety, and may need to make changes to the method of preparation or the dish in line with municipal and federal guidelines.

The following question helps us to prioritize who will participate in the  program. Please answer accurately and to the best of your ability.

This information will be kept confidential by The Depot.If we use this information for making reports and such, it will always be in aggregated and anonymous format (ex: 60% of our participants are under 50 years old, or 40% are women, etc...).

Photo & video releases

Summary: By selecting YES to one or both of the boxes below, I agree to The Depot taking photos and/or recording videos of me. I agree to The Depot sharing these photos and/or videos, including for advertising and/or fundraising purposes, without receiving any monetary compensation.

Please note: You can participate in all Depot programs without agreeing to this media release; however, we ask that you remind staff that you do not want your picture taken if and when someone is taking pictures or videos.

Data collection consent

I agree that The Depot can collect and use my personal information in order to:

• Manage my (and my household’s) involvement in The Depot’s activities, including emergency food access, workshops, volunteering, other programs, and donating funds;
• Communicate with me about these activities, including making me aware of connected opportunities;
• Develop statistics and reports, which will be anonymized (no one’s personal information is included) and aggregated (using large numbers of people so that no individuals can be identified).  These are used to improve programs, seek funding, and develop advocacy tools.

I understand that I have the right to access the personal information that The Depot has collected from me, to correct it if needed, and to ask for it to be removed from The Depot’s system at any time. I understand that I do not need to give The Depot my personal information, but this may have an impact on how The Depot services are delivered to me.

 

If you are not selected for the Guest Chef program, The Depot will delete the information you submitted in this form one year later.
