Candidate Information Form
The Candidate Information Form provides information about you and your campaign to members and voting delegates. This information, along with your one-page flyer, will be included in a Candidate Information Booklet that will be distributed to all attendees at the District Educational Conference. This form must be electronically submitted by February 13, 2026. In addition, your service agreement and candidacy forms must be mailed or emailed to the District Administrator by February 13, 2026 to be an official candidate at the District Educational Conference. You will receive an email receipt upon successful submission of this form.

Your campaign flyer must be a single 8.5x11 page, uploaded in the form of a .PDF File. Campaign flyers are distributed in the Candidates Book (printed in black-and-white) to all DECON attendees. We recommend designing your flyer in black-and-white with a white background to make sure that the colors will be easily readable when printed.
