My child/children are fit and well and should the medical condition of my child/children change it is my responsibility to inform Netball 4 All before they participate in any arranged activities with them.I give my consent that if an emergency medical situation arises, Netball 4 All may act in loco parentis for administration of first aid and/or other medical treatment that in the opinion of a qualified medical practitioner may be necessary.
