The application must be completed by the candidate, approved by the Residency Program Director, and submitted with the necessary documents as stated hereunder. NOF will accept one application per award. All applications must be received no later than the date specified, of a given year, for consideration. Each application will receive an acknowledgment letter. The award(s) will be announced by NOF at a specified date.
The submission requirements include:
1. Application Form
2. Letter of recommendation from Residency Program Director on program letterhead
3. Curriculum Vitae
4. Personal narrative
5. Submission of an Interesting Neurosurgical Case
The personal narrative should include at least the following:
1. Information on the applicant, the community they are from, and the training program.
2. Information regarding how they plan to use the knowledge gained from participating in their chosen course/fellowship to help the medically underserved population in his/her community.
The Interesting Neurosurgical Case will be presented at the Skull Base Course in a case presentation format including history, neurological exam, case imaging, operative pictures or video, and outcome. For the application, a brief (1-2 paragraphs) describing the case as well as the reason why you think that it is an interesting case worth presenting.
Any application submitted with insufficient information/documentation or improperly completed will be returned for revision (if time permits) and may not be included for consideration for the given year. Applications from previous awardees will not be considered.
Keep a copy of the application for your records.
