Knowing the physical and health condition of myself / my child, I give my approval of my / their participation in the New Hope Worship Center basketball program. I hereby assume all risks and hazards incidental to the conduct of the activities and transportation to and from such events. I certify that I am / my child is covered as indicated below. I will not hold New Hope Worship Center responsible for injury or accident.

Heavenly Hoops Code of Conduct

The purpose of Heavenly Hoops basketball league is to provide a safe, pleasant and joyful environment for players, coaches, referees, staff, parents, family and fans to enjoy. In order to achieve this, we need everyone working together to accomplish this. Everyone who participates and watches the games need to be aware of their actions and attitudes at all times. The issues that we'v had over the years seem to come from the adults watching the games, getting upset at a call the referee made or a decision the coach has made. We are asking everyone to be aware of these type moments and not allow your emotions and comments get out of control. Please know that the referees will not always make all the correct calls, although they will do their best. Keep in mind that the Heavenly Hoops staff, coaches, referees are volunteers who are giving their time to produce a healthy, safe environment for everyone to enjoy. Therefore, we are asking each parent/guardian to be responsible for the conduct of those watching your son / daughter/ player. If needed, please address your family member or friend immediately who is not adhering to creating a positive environment we desire to maintain. If we all work together we can maintain a great exciting environment. Thank you for your understanding and cooperation!

1.I will exhibit good sportsmanship behavior at all times. No physical or verbal intimidation of any individual. No cursing, abusive language, trash talking, or yelling directed towards another person. Abusive language is considered any communication, verbal or physical (a.k.a. body language), that would commonly be considered vulgar, abusive, demeaning or otherwise inappropriate. No fighting, punches, elbows, kicking, head butting or any other form of physical assault will be tolerated. 2. I will act as a positive role model by showing positive support for all fellow players, coaches and game officials, treating other players as I would like to be treated 3. I will refrain from criticizing game officials at all times. I will control my temper and refrain from arguing with my coach or a referee. I will restrain myself and not retaliate even when I feel wronged. I will win and lose with dignity. 4. I will keep my shorts / pants pulled up to my waistline at all times. 5. I will not consume or distribute alcohol and /or tobacco products and /or any illegal substance of any kind. 6. I will not physically damage or steal items from this facility. (Restitution will be required 7. I will not bring to this property fireworks, weapons or any item or material which would be a hazard or harmful to other persons. *Only concealed carry firearms are permitted on the church property. 8. If you do not have a home church you are welcome at New Hope Worship Center. Sundays: Sunday School 9:30am, Morning Worship 10:30am, children's church 10:30am. Wednesdays: Royal Rangers/Mpact 6:30pm. Adult Bible Study 6:50pm

I agree to adhere to this Code of Conduct. Failure to comply to with this Code of Conduct and any other verbal instructions given by Heavenly Hoops staff will be grounds for immediate dismissal from the league. By signing this Code of Conduct you acknowledge that you have read, understand and agree as stated.

NAME OF EVENT: Heavenly Hoops Basketball League November 2024 - March 2025

I acknowledge that basketball or any athletic event is an extreme test of a person's physical and mental limits and that my participation in a basketball event carries with it the potential for death, serious injury and property loss/damage. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of athletes, equipment, vehicular traffic, actions of other people including, but not limited to, participants, volunteers, spectators, coaches, event officials, and event monitors, and/or producers of the event, and lack of hydration. These risks are not only inherent to athletics, but are also present for volunteers. I hereby assume all of the risks of participating and/or volunteering in this event. I realize that liability may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them or because of their possible liability without fault. With full understanding of the potential risks, I HEREBY ASSUME THE RISKS OF PARTICIPATING IN ANY WAY IN A BASKETBALL EVENT. I certify that I am physically fit to participate in this event and have not been advised otherwise by a qualified medical person. I acknowledge that this Accident Waiver and Release of Liability (AWRL) form will be used by the event holders, sponsors and organizers, in which I may participate and that it will govern my actions and responsibilities at said events. I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: I WAIVE, RELEASE AND DISCHARGE from any and all liability for my death, disability, damage, property theft or actions of any kind EXCEPT THAT WHICH IS THE LISTED BELOW, which may hereafter accrue to me or my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: NEW HOPE WORSHIP CENTER their directors, officers, employees, volunteers, representatives, and agents, the event holders, event sponsors, event directors, event volunteers; INDEMNIFY AND HOLD HARMLESS the entities or persons mentioned in this paragraph from any and all liabilities or claims made by myself, other individuals or entities as a result of any of my actions. I consent to receive medical treatment which may be deemed advisable in the event of injury, accident and/or illness during this event. I understand that at this event or related activities, I may be photographed. I agree to allow my photo, video or film likeness to be used for any legitimate purpose by the event holders, producers, sponsors, organizers and/or assigns. This AWRL shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I hereby certify that I have read this document, and, I understand its content. This AWRL shall apply to each and every time I participate in this event from December 2024 through March 2025.

RESULT OF GROSS NEGLIGENCE AND/OR WANTON MISCONDUCT OF PERSONS OR ENTITIES

The undersigned parent and natural guardian or legal guardian of the applicant ( for and on behalf of the minor named herein. I hereby bind myself, the minor and all other assigns to the terms of the Waiver and Release. I represent that I have legal capacity and authority to act for and on behalf of the minor named herein, and I agree to indemnify and hold harmless the persons or entities named in the Waiver and Release for any claims or liabilities assessed against them as a result of any insufficiency of my legal capacity or authority to act for and on behalf of the minor in the execution of the Waiver and Release. I have also read and understand the AWRL above and have reviewed them with my child (participant I fully consent to my child's participation in this New Worship Center Heavenly Hoops Basketball event.
