1 To 1 SESSION TRIAL REGISTRATION
Welcome to NLP Football Academy. Please note this form is for 1 to 1 TRIAL registrations only. This form allows players to attend sessions for the specified days only. Once submitted you will receive a text confirmation with all the registration details and coaches information to make final arrangements i.e. meeting points and ascertain any requirements.

Full registration can be made for weekly bookings and only players named on this registration form will be permitted to attend and be added to the coaches register (1 player per registration). If any payments fail to go through the session will be cancelled so please ensure you are up to date with the agreement. This form must be completed at least 7 days BEFORE and not DURING your first session to be confirmed or the coach has the right to refuse attending. Please note 1 to 1 payments are non refundable although it can be transfered to a later date if you are unable to attend on your registered date.
