    First-Time Homebuyer & Pre-Purchase Counseling
  • Thank you for reaching out to the Northwest Center. We are a HUD-certified, non-profit organization that offers FREE counseling. Please complete this intake form to proceed with your Pre-Purchase counselor.

  • Expectations & Understandings

  • SCOPE OF NWSHC SERVICES

    1. The NWSHC has a successful track record of educating homeowners, renters, and others to achieve their financial education. Goals of increasing credit scores, reducing debt, and creating a realistic budget are often met through the process, but not guaranteed.
    2. While the NWSHC may be able to explain various legal processes, we do not provide legal advice and you are encouraged to meet with a licensed legal professional should you feel it necessary.
    3. NWSHC employees, board members, contractors, and any others connected to the organization CANNOT accept personal gifts from clients. If you feel compelled to give a donation, please consider a tax-deductible donation to the organization.
    4. You are NOT obligated to receive or participate in any other NWSHC services or programs.

    UNDERSTANDINGS & EXPECTATIONS

    1. You are expected to be 100% honest in in the completion of all forms and in disclosure of your situation with us. If you are not honest, we reserve the right to close your file.
    2. You are expected to respond to communication attempts on our behalf within 24-48 hours (during business days). We will respond within the same timeframe.
    3. Per HUD guidelines, we are required to follow up with you monthly, or every 60 days. If we do not hear back from you (three attempts over a six- week period) we will close out your file.
    4. You are required to inform us if you cannot make an appointment. Every no call/no show appointment we experience is a missed opportunity for another client.
    5. Your Financial Coach will determine the time frame of your scheduled time together per your goal, please be mindful of both our time and yours.
    6. We do not provide any funds toward your home purchase. Set aside as much money as possible toward your personal resolution.

  • NORTHWEST SIDE HOUSING CENTER (NWSHC), an Illinois not-for-profit corporation, would like to advise you of its privacy policies. NWSHC has collected non-public personal information from your application and consumer reporting agencies.  This non-public personal information includes your address and other contact information, demographic background, application status, family income, social security number, employment information, collection and repayment history, and credit history.

    All employees of NWSHC at all times will maintain the confidentiality of its clients, participants and volunteers. We disclose non-public personal information to third parties: only as necessary to process and service your application and forgivable loan; only as necessary to effect, administer or enforce your forgivable loan; with your consent; or as permitted or provided by applicable laws, including the Illinois Freedom of Information Act (“FOIA”) and the Privacy Act of 1974.  Applicable laws permit disclosure to third parties for certain purposes. Examples of such disclosures include (i) disclosure in connection with enforcement purposes or litigation, audits or other investigations; (ii) to comply with proper requests under FOIA or other federal, state, or other local laws and regulations; and (iii) to federal and state agencies to the extent specifically permitted or required by law. We do not sell or otherwise make available any information about you to any third parties for marketing purposes.

    We protect the security and confidentiality of non-public personal information by limiting and monitoring all physical access to sites where non-public personal information is kept. A complete copy of our written privacy policy is available upon request.

    If we decide to change our privacy policy, we will provide you with a revised privacy policy containing such changes.

  • Consumer Credit Authorization Release

  • I hereby authorize CoreLogic Credco LLC (“CREDCO” or “FAC”) and/or  TransUnion Direct, to obtain my consumer report/credit information, credit risk scores and other enhancements to my consumer Report (hereinafter collectively referred to as “Report”) from one or more of the three national credit repositories (Equifax, Experian, Trans Union) and provide a copy of the Report to my housing counseling agency, Northwest Side Housing Center (“Counselor”) for Counselor to provide housing counseling services. This authorization is intended to comply with a consumer report request as set forth in 15 U.S.C.1681b(a)(2).

    I acknowledge that the Report is provided “AS IS” and that Credo and Transunion Direct makes no representation warranty, express or implied, including, but not Limited Implied warranties of merchantability or fitness for a particular purpose and implied warranties arising from a couse of dealing or a course of performance with respect to the accuracy, validity, or completeness of the report or that it will meet my needs and Credco expressly disclaims all such representations and warranties. 

    I recognize that the accuracy, validity or completeness of the report provided by CREDCO and TransUnion Direct is not guaranteed and I hereby release CREDCO’s and TransUnion Direct’s parent, sister, affiliated companies, successors and assigns and its and their directors, officers, agents, employees and independent contractors (collectively, “CREDCO’s Affiliates” “TransUnion Direct’s Affiliates”) from any liability for any negligence in connection with the preparation of the Report and from any loss, damages, expenses, costs or obligations of any kind and nature whatsoever suffered by me resulting directly or indirectly from the inaccuracy, invalidity or incompleteness of the Report.

    I covenant not to sue or maintain any claim, cause of action, demand, cross action, counterclaim, third party action or other form of pleading against CREDCO and/or TransUnion Direct and their affiliates for damages based upon the inaccuracy, invalidity or incompleteness of any Report provided by CREDCO and/or TransUnion Direct hereunder.

    If one or more of the provisions, or a portion of a provision of this document are held for any reason to be invalid, illegal or unenforceable, such invalidity or illegality or unenforceability will not affect any other provisions of this document, and this document will be construed as if such invalid, illegal or unenforceable provision had not been contained herein.
