Gender Identity EO and Immigration Documents Reporting Form
Immigration Equality and Oasis Legal Services are collecting examples of USCIS and DOS not issuing documents with the correct gender marker (based on self-attestation or court order). If you have not previously reported an issue, please do so using this JotForm. You may have previously reported an issue using this Google form: https://forms.gle/U1rWSf7wYAennoLu5 We would like to receive more information and documentation regarding your situation. This documentation will be securely stored by Oasis Legal Services, and shared with our partner Immigration Equality. The purpose of collecting documentation is to be prepared for potential litigation or administrative advocacy in response to the administration's actions. Your documentation may be shared with litigation partners for the purpose of exploring litigation options. Litigation or other advocacy would be initiated to ensure that everyone can access immigration documents and passports which accurately reflect their identity.

Documents - please provide documentation regarding your experience. For example, if you submitted an application for a DHS benefit, such as an I-765, I-485, or N-400, please upload a scan of the form which you submitted. If you received a document from DHS or DOS, such as a work permit, green card, naturalization certificate, or passport, please upload a scan of the document you received. If your new document doesn't match old documents, please provide photos/scans of the old documentation. If you received any notices from an agency regarding this issue (e.g. receipt doesn't match issued document or approval notice, request for evidence (RFE), or denial notice), please upload a scan of that agency correspondence. If you provided supporting documentation to the agency, such as an amended birth certificate or court ordered name and/or gender marker change, please submit a scan of the document.
