I have done neither of the above listed options. My child(ren) will not be able to be registered as a student in the OASN K-12 Education Division.
By signing below, I acknowledge that my child is enrolled as home education student in the county I reside in & with the OASN K-12 Education Division (you must have the Unique Abilities scholarship or cash pay to be eligible for this academic program). This means that I am responsible, along with my child, to ensure that academic work assigned is being completed within one week (please note: you are contracting with OASN to provide academic instruction while onsite at OASN 7634 SW 60th Avenue, Ocala, FL 34476). I further understand that at the end of the academic year, I am responsible for submitting standarized testing to my county's home education office (OASN will provide this to parent) or that I will contract someone to complete a home education evaluation (at parent expense). I understand that I am exempt from the Absence Policy (your child is marked hybrid on the days not onsite) but I am still responsible for doing academic work with my child when he/she is not receiving instruction onsite at OASN 7634 SW 60th Avenue, Ocala, FL 34476.
