  • Appointment Preparation Form

    Please complete and submit this form prior to arriving to your appointment.
  • Cancellation Policy

    It is required to cancel/reschedule your appointment at least 48 hours prior to your appointment. Cancelling or rescheduling within 48 hours will result in your deposit being nonrefundable with the option of transferring your deposit to your next scheduled appointment.

    Cancelling within 48 hours results in a 50% service charge fee. Cancelling on the calendar date of your appointment results in a 100% service charge fee.

    Failure to complete payment results in not being scheduled any future appointments with us.

     

    No Show Policy

    No Shows are not tolerated and your deposit will not be refunded or transferred. If a No Show occurs, it is up to our discretion whether or not to book you any future appointments. Cancellations on the calendar date of your appointment will be treated as No Shows.

    No Show fees are 100% of your scheduled service and are required to be paid to your service provider.

    Failure to complete payment results in not being scheduled any future appointments with us.

  • Service Adjustment Policy

    Service adjustments can be made up to 48 hours prior to your appointment. Any adjustments made within 48 hours of your appointment that involve a lower price value than your original service price will require a payment of 100% of your original service price.

     

    Late Arrival Policy

    Arriving on time is essential to receiving the complete benefits of your service with us. If you show up late to your appointment, it is up to our discretion to make adjustments to your service or reschedule. 100% of your original scheduled service price will be issued no matter what adjustments are made.

     

    Booking Policy

    Some services will require a nonrefundable deposit to book the appointment. This amount will go towards your service at the end of your appointment.

    In cases when an appointment needs to be rescheduled, deposits are transferable up to 2 appointment adjustments. After that, a new booking deposit will be issued.

     

    Payment Policy

    Please contact your service provider for their accepted payment options. For any card payments, please expect an additional processing fee. Payments must be completed in full during your visit.

  • Small Children Policy

    To maintain a calm, peaceful, and safe environment for our service providers and clients, please refrain from bringing small children with you to your appointment with us.

    Our spa holds potential hazards for a child left unattended and if injured, Ocean Jasper Wellness and our insurance company will not be held responsible.

    If you arrive with a small child, we may ask you to reschedule your appointment with us.

    We love children and have our own, but we are here to pamper you and appreciate you making other arrangements for your children while you are with us.

  • I agree not to visit Ocean Jasper Wellness for any of the services provided if I have any symptoms of COVID-19, have been exposed or diagnosed with COVID-19 in the last 14 days or currently live with someone who is experiencing symptoms or in quarantine because of COVID-19. I acknowledge that the information I have given in this consent form is accurate and complete. 

     

    I verify that the information I have provided on this form is truthful and accurate. I have read, understand and have checked all terms and conditions. I release Ocean Jasper Wellness and all service providers and parties related to Ocean Jasper Wellness from any and all of the claims and demands associate with its services.
