Mission Statement of the Ontario Federation for Cerebral Palsy

The Ontario Federation for Cerebral Palsy (OFCP) is a non-profit, charitable organization that strives to address the needs of people with cerebral palsy in the province of Ontario.

- Incorporation, Letters Patent (if your organization is incorporated)

- Description of programs, services provided and/or facility operated
- Percentage of the people you serve who have cerebral palsy
- List of all groups/organizations you are affiliated with
- Copy of the motion of your Board of Directors pertaining to this application

Eligibility for Group Membership

Any group interested in helping people with cerebral palsy or other disabilities.

Together we can be a strong voice. Add your own perspective, and help us influence policy, public opinion and the availability of supports for people with disabilities in Ontario.

Membership Fees

A membership fee of $100.00 is required with the completed application payable to the Ontario Federation for Cerebral Palsy or OFCP.
