By checking this box I acknowledge my appointment request may be denied even after completion. I may be referred out for the services that I am seeking. If a service I am seeking is not available for selection , then that service may not be offered by the artist.
I acknowledge I will have to place a $100 retainer fee to secure my date and time if my appointment is approved. The retainer fee will go towards drawing time and the end cost of my tattoo. I accept that the hourly rate of the artist is $200 with a minimum of $170. This rate is subject to change by discretion of the artist. Flash events and special flash deals are exceptions to the artist’s hourly rate.

I acknowledge larger tattoos result in multiple sessions. I understand large tattoos are a commitment to see completed up until the end.

I acknowledge hate , racist symbols , gang related imagery, or phrases of any kind will result in automatic denial of application. I accept the artist’s refusal of any future work with me if I ask for such type of tattoos.
