Columbia Theological Seminary welcomes open communication from students regarding its policies and practices. Student feedback helps members of the administration, faculty, and staff evaluate, clarify and improve processes and procedures.
Please review the Student Complaint Policy (Student Handbook, p. 21) before submitting this form. This form is intended to be used by students to submit a formal complaint about a matter they have been unable to satisfactorily resolve with a faculty, staff, administrator, or other students involved.
This tool should not be used for emergency or crisis situations needing an immediate response from public safety or law enforcement officials. Depending on the amount of information detailing the alleged incident(s) or identifying the alleged individual(s) responsible, CTS's ability to respond may be limited.
This tool should not be used to report incidents that involve harassment, discrimination, bias, violence, sexual assault, or sex/gender-based assault, harassment, or misconduct.
