In the event of an emergency, I understand Ozzy & Friends LLP will make every attempt to contact the Owner and Emergency contacts listed on the registration form. In the event that no contact can be reached, I authorise Ozzy & Friends LLP to seek appropriate medical care for my Dog(s).

I understand that every effort will be made to take my Dog(s) to the Veterinarian listed on the Registration Form, however, I authorise Ozzy & Friends LLP to seek treatment for my Dog(s) at any appropriate clinic, if necessary.

I authorize Ozzy & Friends LLP and the Veterinarian caring for my dog(s) to share all medical records of my dog(s) with emergency vet clinics in an effort to provide the best care possible.

I agree to assume full responsibility for payment and reimbursement for any and all veterinary services rendered.

I authorise Ozzy & Friends LLP to transport my dog(s) to and from veterinary clinic for treatment or to request “on-site” treatment if deemed necessary.

I understand that Ozzy & Friends LLP assumes no responsibility for the loss or injury of any dog(s) and is released from all liability related to transportation, treatment, and expenses.

This agreement is valid from the date below and grants permission for all future veterinary care without additional authorization each time the Care Provider cares for my dog(s)
