RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISK & INDEMNIFICATION AGREEMENT
Dance Images.LLC Convention – August 22, 2026
By signing this Agreement, the undersigned participant (and parent/legal guardian if participant is under 18 years of age) acknowledges and agrees to the following:

1. Participation and Assumption of Risk
I understand that participation in dance classes, workshops, rehearsals, performances, competitions, fitness activities, and related convention activities involves inherent risks, including but not limited to bodily injury, illness, falls, collisions, physical exertion, emotional stress, property damage, permanent disability, and death.

I voluntarily choose to participate in the Dance Images.LLC Convention event taking place on August 22, 2026 Empower Dance Convention By Dance Images.LLC and knowingly assume all risks associated with participation, whether known or unknown, foreseeable or unforeseeable.

2. Release and Waiver of Liability
To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue Dance Images.LLC, its owners, officers, directors, employees, contractors, instructors, staff members, volunteers, representatives, agents, affiliates, sponsors, vendors, venue operators, and property owners where the Event is held (collectively, the “Released Parties”) from any and all claims, liabilities, demands, causes of action, damages, losses, costs, or expenses of any kind arising out of or related to my participation in the Event, including claims arising from negligence of the Released Parties.

This release includes, without limitation, claims for:

Personal injury
Illness or medical conditions
Property loss or damage
Emotional distress
Economic loss
Wrongful death
Any other damages arising from participation in the Event
3. Indemnification
I agree to indemnify, defend, and hold harmless the Released Parties from and against any claims, actions, suits, damages, liabilities, costs, or expenses, including attorney’s fees, arising out of my participation in the Event or caused by my actions or conduct.

4. Medical Authorization
In the event of injury or medical emergency, I authorize Dance Images.LLC and Event personnel to secure emergency medical treatment on my behalf if necessary. I understand that I am solely responsible for any medical costs incurred.

5. Photography and Media Release
I grant Dance Images.LLC permission to photograph, video record, and use my image, likeness, voice, and performance for promotional, advertising, marketing, social media, and educational purposes without compensation or further approval.

6. Rules and Conduct
I agree to comply with all Event rules, policies, instructions, and safety procedures. Dance Images.LLC reserves the right to remove any participant for unsafe, disruptive, or inappropriate behavior without refund.

7. Minors
If the participant is under 18 years of age, the parent or legal guardian signing below acknowledges that they are signing on behalf of the minor participant and agree to all terms of this Agreement individually and on behalf of the minor.

8. Governing Law
This Agreement shall be governed by and interpreted in accordance with the laws of the State of New Hampshire.

9. Severability
If any provision of this Agreement is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect.

10.Payment I acknowledge that in order for my student to be fully registered payment in full is required.

11. Acknowledgment
I HAVE READ THIS AGREEMENT CAREFULLY AND FULLY UNDERSTAND ITS TERMS. I UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO FILE A LAWSUIT AGAINST THE RELEASED PARTIES. I SIGN THIS AGREEMENT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Medical Release and Authorization

As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to Dance Images.LLC . and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered season.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.
