  • Parent/Guardian Release (for minors under 18)

    I hereby am signing this Release and Waiver, and acknowledging all provisions contained within, as a parent or legal guardian for the minor listed below. I recognize and accept that all provisions of this Release and Waiver, including the inherent risks of the event or activity, apply to minor participants as well as adults. I also recognize that I am not only signing as a parent/guardian, but also for myself as a participant.

  • Release & Waiver of Liability

    In consideration of participating in events and activities with The Hatchet House LLC, or by its employees, members, managers, officers, volunteers, agents and/or assignees (collectively “The Hatchet House”), both at The Hatchet House LLC’s location or at offsite events, the undersigned acknowledges and agrees that:
    1. I am aware of the risks associated with the activity I am about to engage in; specifically, risks of serious bodily injury, property damage and death. I recognize that, even with precautions, The Hatchet House, like every axe throwing venue, cannot predict the outcome of every thrown axe or hatchet, and serious risks exist for all participants. I accept these risks as a condition of my participation.

    2. I knowingly assume all risks, both known and unknown, even if arising from the negligence of The Hatchet House, other participants, or other guests.

    3. I am not under the influence of any substances, including alcohol, illicit or prescription drugs, which may affect or impair my coordination, judgment or ability to safely participate in this activity.

    4. I am healthy and can safely participate in this activity and not endanger myself or other participants or guests. I do not have any physical, emotional, or mental conditions or deficiencies that would negatively affect my judgment or physical participation in this event or activity.

    5. I am aware of the rules of participating in this event or activity safely, most notably; only one axe throwing participant in the designated axe throwing zone for each lane at a time (inclusive of the axe throwing target area), no person to person handoffs with axes/hatchets, and always listen to the instructions of The Hatchet House LLC Employees often referred to as “Axeperts.”

    6. I authorize The Hatchet House to terminate my participation in the event or activity at any time, without refund, in the sole discretion of the employee, member, manager, officer, volunteer, agent or assignee, if my conduct has been deemed unsafe for myself, other participants, guests, or employees, or if I am in violation of any of the rules of this Release and Waiver, or the rules of the event or activity.

    7. In the event that medical treatment may be necessary, as a result of my attendance and/or participation in the event or activity, I authorize and give my consent to The Hatchet House to secure treatment, including, if necessary, emergency transportation and emergency surgery, and I assume all financial responsibility and any other costs related to the medical problem, including but not limited to the treatment and transportation. I give my consent to The Hatchet House to disclose any known medical information as part of any treatment.

    8. I hereby authorize and give my consent to The Hatchet House to capture my image and likeness in photographs, videotapes, digital recordings, and any other forms of media. I understand that this media will be property of The Hatchet House LLC. I release The Hatchet House to copyright, display, distribute, use, modify, print, reprint, and publish this media by any legal means, including but not limited to, social media marketing, publications, advertisements, websites, pamphlets, brochures, and television and radio commercials. I irrevocably waive any claims related to the use of images or media captured during my participation of events at or with The Hatchet House.

    9. I authorize The Hatchet House to contact me by phone, e-mail, or text at the information provided with this Release and Waiver, to confirm reservations and to offer future promotions or advertisements. I understand that I can opt-out of texting or e-mailing at any time by replying “stop” to texts and by following the e-mail instructions to unsubscribe.

    10. I, for myself, my heirs, successors and assigns, hereby release, waive, discharge, and agree not to sue The Hatchet House LLC, its members, officers, employees, volunteers, agents, assignees, representatives, subsidiaries, successors and assigns, for any injury, death, loss or damage to person or property, regardless of the presence of negligence of or by its members, officers,
      employees, volunteers, agents, assignees, representatives, subsidiaries, successors and assigns, to the fullest extent permitted by law. I further agree to indemnify and hold harmless The Hatchet House LLC and The Hatchet House from any loss, liability, damage or cost they may incur, regardless of negligence of or by its members, officers, employees, volunteers, agents, assignees, representatives, subsidiaries, successors and assigns.

    11. I agree that this Release and Waiver will be governed by the laws of the Commonwealth of Pennsylvania. If any provision of this Release and Waiver is deemed unenforceable that shall not affect the enforceability of the remaining provisions of this Release and Waiver.

    When a participant reaches the age of 18, a new waiver is required, and the previous guardian-signed waiver is no longer valid.

    This waiver will be kept on file; you can request a copy or revoke this waiver by mail addressed to The Hatchet House, 362 Market St., Williamsport, PA 17701.

    I have been given ample time to read this Release of liability and Waiver and fully understand all terms contained within, as well as the risks associated with this event and activity.
