Parasport Denmark requires this Declaration of Compliance of all participants to limit the further outbreak of COVID-19. This Declaration of Compliance will be kept safely, and personal information will not be disclosed unless as required by law or with your consent.

A participant, who is unable to agree to the terms outlined in this document, is not permitted to participate in the 2022 Wheelchair Rugby World Championship. A Team Manager is allowed to give consent on behalf of the whole team of participants. In this case, the Team Manager is responsible for informing the team of the content of the Playbook of the 2022 Wheelchair Rugby World Championship and the team’s compliance of the Playbook.

I, the undersigned being the participant named above hereby acknowledge and agree to the terms outlined in this

1) The participant has not been diagnosed with COVID-19; OR if the individual was diagnosed with COVID-19, the participant has a positive test. This test must be 5 days or more on the departure date, and he must be without symptoms before travelling to Denmark.

2) The participant is attending or participating voluntarily and understands the risks associated with COVID-19. The participant agrees to assume those risks, including but not limited to exposure and being infected.

3) The participant is not currently experiencing any signs or symptoms of COVID-19 (including fever, new or worsening cough, fatigue, chills and body aches, respiratory illness, difficulty breathing, nausea, vomiting or diarrhea, or loss of taste or smell) OR if the individual is experiencing mild symptoms as above it has been at least 5 days since the onset of symptoms or a positive test in the case of no symptoms.

4) If a participant experiences any signs or symptoms of COVID-19 after submitting this Declaration of Compliance, the participant will follow the guidance of the Organizing Committee AND will immediately report those symptoms to their Team Manager (Teams) or the COVID Manager AND isolate while awaiting further instructions. Team Managers agree to report all symptoms to the COVID Manager immediately.

5) The participant is following recommended guidelines and protocols of the Provincial Government and the Organizing Committee.

Furthermore, by signing below, the participant agrees that while attending or participating in the Organization's events or attending at the Organization's facilities, the Participant:

1) Will follow the guidelines of the Playbook

2) Acknowledges that the Organization may remove the participant from the facility or from participation in the activities, programs, or services of the Organization at any time and for any reason if the Organization believes, in its sole discretion, that the individual is no longer in compliance with any of the standards described in this document.

3) Will guarantee that a photo or photocopy is of his/her own self-administered test.
